What Does Fitness Mean, Part Two

Heart disease can be potentiated by stress as during stress the adrenaline secretion forces the blood vessels to narrow which increases blood pressure and venous return to the heart, thereby increasing demands on the heart and leaching cholesterol out of the tissue into the blood supply. Stress can maintain the activity in the reticular activating system or RAS, elevating coronary disease risks and the tendency to react with anger. Exercise can reduce the risks if managed correctly.

Chronic Segmental Low Back Stiffness

When we have our first episode of low back pain our back is rarely stiff unless we are just like that, having a tendency towards stiffness. Mostly our back moves like the well designed machine it is. The discs separate the vertebrae and allow movement to occur, with thicker discs in areas of greater movement. The facet joints control and limit movement of the segments and prevent abnormal displacement of the spine under the shearing loads it must cope with. The muscles are all in good condition and working in the most functional patterns. Until, at least, an episode of back pain occurs and it all changes.

What Does Fitness Mean?

In the springtime we all start thinking about how we could be slimmer, fitter and look better for the summer, if we haven’t already taken up a gym membership in January and let it lapse fairly quickly as the first flush of enthusiasm faded. Through all this most of us do not stop and think about what fitness actually means to us. Being fit is a desired state but this state is not defined and we have little idea what all the measurements of fitness mean, all of which makes it less likely that our rather vague plan will be ultimately successful. Physiotherapists may have a somewhat clearer grasp of some of the concepts but most will not be very precise.

What Does It Mean To Be Fit?

The early part of the year is the time for the new resolutions we have about many things, including our personal appearance and fitness. Summer will soon be here and we want to look our best. Perhaps we have already signed up for the gym and rather predictably not attended anywhere near as much as initially intended. We want to be fit but are not clear at all what that means or the significance of the typical fitness measures used to track progress. Our lack of knowledge impairs our ability to plan our training and reduces the chances of success. While physiotherapists understand more about these concepts than most people they are still not that knowledgeable in detail.

The challenge of fractures through the ages has been how to manage the severe pain, immediate disability and long term sequelae of these acutely presenting emergencies, making broken bones clear priorities due to the large number of incidences. The variety of fracture treatments includes traction, joint replacement, immobilisation, amputation and internal fixation. Open fractures with significant soft tissue injury and damage were and remain at risk of infection which was commonly treated with amputation in the past. Ice and Cold Therapy

Cold therapy or cryotherapy is a common and useful therapeutic modality often used by physiotherapists in treating a wide range of conditions. It is easy to apply and if care is taken over cautions and contraindications it is very safe and patients can be instructed to self treat to manage their conditions independently. Cryotherapy is most commonly used in sports and acute injury treatment and is cheap and simple to use. Cold can be applied in several different ways including cold packs, crushed ice, cubed ice or cold water devices.

BUPA’s Shock Pressure on Private Physiotherapy

BUPA’s introduction of its recent tender process has caused anger and grave concern amongst private physiotherapists. BUPA is the largest private medical insurer in the UK and has been reviewing orthopaedic services recently, now moving on to the reported inequalities in private physiotherapy services. BUPA’s intentions are stated to be to look at the alleged inequalities in quality, cost and customer service in physiotherapy provision across the country. Six thousand physiotherapists have been effectively requested to enter a price competition to be allocated BUPA patients.

Fractures of the Femoral Neck

Neck of femur fractures are one of the commonest fractures in the population as bone density in the thigh area drops in elderly women after the menopause, with large numbers of hospital beds dedicated to this group. Abnormal stresses repeatedly applied to the neck of femur such as by long distance runners or military trainees can result in stress fractures of the femoral neck in this much fitter and younger group. Direct trauma to the area such as a fall on the side of the hip can cause fracture at any age as can fractures secondary to pathological changes such as neoplasm.

Diagnosing Low Back Pain

Low back pain is one of the most troublesome and disabling musculo-skeletal conditions with very high costs and diagnosing it is tricky due to the large number of potential pain generators. Back pain is not one condition but a variety of inter-related syndromes which have been characterised as back pain subtypes, the identification of which may be crucial to effective intervention. The large number of potential diagnoses includes lumbar stenosis, pain of discal origin, trigger point syndromes, postural pain, facet joint syndrome, nerve root pain and neuropathic pain.

Heat Treatments

Superficial heat treatments are very commonly used for aches and pains by the public although rather less so by physiotherapists nowadays. These types of treatment do not heat deep into the body so cannot affect muscles directly as the subcutaneous fat layer below the skin functions as a heat insulator. Superficial heating also causes increased blood flow through the skin of the heated area, which carries off some of the heat being applied. The methods of heating are varied and are generally described as heat conduction, heat convection and heat conversion.
